For many, the challenges of caring for a family member are simply a part of daily life. Caregiving, though, is a demanding job and no one is equipped to do it alone. Respite care provides short-term breaks that can relieve stress, restore energy, and promote balance in your life.

We offer truly personalised respite options including supporting individuals in their own home, an environment they know and are already comfortable with. As our services are life long, it avoids the stress of unnecessary transitions and ensures continuity in how a person’s support is delivered.

New Horizons are Approved Providers of Short Breaks Respite Services for Adults and Home Care Services with The States of Jersey and can be funded privately or if arrangements are in place through Health & Social Services funding. Supporting You 
Whether your care is paid for by private or public funds, we can deliver a high-quality service to you, tailored to your individual requirements. Our services are accessible to individuals with various additional and complex needs and can be funded privately or if arrangements are in place through Health & Social Services funding.
 

Outreach Respite Services

Our respite services are for Individuals whose carers, parents and family members wish to have some well needed rest by going on an overnight break or on holiday, whilst leaving the individual in their own home environment supported by experienced and caring staff, rather than being placed in a more formal care setting. 


Should you wish to have a shorter period of respite, for example, just an afternoon break twice a week where you’re free to spend time with friends, start a hobby or just have some time to yourself, 


New Horizons can provide a Support Worker who can look after all of your dependent’s care needs whilst you take time away from your everyday care duties to relax, go on holiday or just have a few hours break.


We understand that being a regular Carer for a loved one can make it difficult to arrange some respite for yourself and you may feel apprehensive about going away. New Horizons can provide a high-quality care service, affording you the peace of mind to relax and enjoy yourself, knowing that your loved one will continue to receive the care and support they need.

Community Support Services 

New Horizons Community Support Services provide a range of support options that enable people to live with independence, access their communities and have opportunities to socialise. 


Our Community Access Support Services provide a personalised alternative to the traditional day services, community outreach and respite services provided on the Island for adults. 


We believe all people should have the opportunity to socialise, access local events and participate in their local community, whilst also continuing their own self-development, regardless of their needs. 


New Horizons Community Support Services provides 1:1 & 2:1 Person Centred support in the community. The Community Support Services we provide are tailor-made around the needs of the individual. 

It can include activities such as shopping, exercise, support and transport to access social events. 

We are able to provide assistance to individuals with additional and complex needs and have trained staff who are able to use the Changing Places Community Care Facilities.


Day Trips Out
In addition to helping you with short day-to-day outings, your Support worker will be happy to accompany you on day trips, allowing you and your family to relax, safe in the knowledge that you are getting the personal attention you require whilst out on your trip.


Whether you are planning a trip to a museum or a trip to a local park, our Support workers will support you whilst you are out of your home. They can also assist you with making travel arrangements and transporting mobility equipment, should you need it.


Social Activities
Wherever possible, we will encourage you to engage in social activities. That may be to attend a local club, to meet with friends for a chat and a cup of tea or going out to a restaurant for dinner. Your Carer will be on hand to support you whenever needed.

Life Skills in the Community

New Horizons Support Services provide life skills support in the community that support people with the opportunity to develop life skills that are required to live independently.


This service is tailor-made around the needs of the individual. It includes life skills assessment & training provided through a structured yet flexible training programme.

 Activities such as shopping, personal finances, travel training, road safety and social skills awareness training are covered to help develop friendships and relationships.


Our support strategies always focus on an individual’s existing interests and abilities and are designed around his or her personal choices. 


Our Support can include explaining why manners are important, what they are, and how and when they should be used, show how what’s “appropriate” is dependent upon the person they’re with; that manners with relatives and friends should be different than those used with neighbours and the general public.


Once the training strategy is developed and implemented, we review it on a regular basis to ensure that the individual is receiving the most effective support, evaluating progress regularly and modifying the plan when necessary in order to optimise learning.


Learning such skills can be a lifeline and the route to a life of independence. We provide training and practice in a variety of life skills targeted towards increasing self-sufficiency in everyday life. Skills are taught in the community to allow for effective real-world learning experiences.

Independent Living Support Services

.New Horizons Independent Living Support Services offers a range of services that provide a support structure & life skills training to individuals wishing to live independently in their own community, who require assistance with their finances, shopping, cleaning and cooking.  Our services provide a personalised option to supported living, enabling people to live on their own or with the people they choose.

Daily Planning
Our Support workers are able to assist you with planning your day to day life, ensuring you have made the necessary travel arrangements to get to you appointments on time. We can provide support with managing your house hold finances so that all your financial commitments are met on time.


Meal Times
As part of our Independent Living Support Service, our Support Workers are able to support you with all aspects of meal times. This can include assisting with your weekly shop, helping with the preparation of your meals, and managing the food in your house.


Household Tasks
Our Support Workers can assist you with light household duties as part of your Independent Living Support Service and will help with tasks such as laundry, vacuuming and washing up. If you are able to complete these tasks yourself in order to maintain your independence, our Support Workers can just be on hand to offer support where required.


Medication Assistance
You may sometimes forget to take your prescribed medication, or may struggle to take it the correct way, which could put your health at serious risk. Our Support Workers are able to alleviate this worry by prompting you to take your medication and ensure you’re taking the right dosage, at the right time.


If remembering to take your medication is not a problem, but you’re unable to physically take it due to your condition, our Support Workers may be able to help administer your medication, provided there is a clear prescription to follow and the medication is dispensed into a MARS Dosset box.
 

Life Skills Support in the Home

New Horizons Support Services provide Life Skills training in the Home that support people with the development of Life skills that are required to live Independently. Learning the basics can be a lifeline and the route to a life of independence. 


Once the training strategy is developed and implemented, we review it on a regular basis to ensure that the individual is receiving the most effective support, evaluating progress regularly and modifying the plan when necessary in order to optimise learning.

New Horizons’ 24/7 Independent Living Support Service provides a support structure & life skills training to individuals living independently in the community who require 24/7 support to do so. 

The service is designed to support you with your day-to-day living, as well as providing you with the encouragement and emotional support that you may need to remain living independently.

The Process

How will you select the right Carer for me? 
We will carry out a full assessment of your care requirements and also get to know your personality and preferences. This process enables us to ensure that we pick a support worker who not only has the relevant qualifications, training and experience but also is someone who you are likely to enjoy spending time with. Once we have identified appropriate support workers to provide your service, we will give you the opportunity to meet with them in order to ensure you are completely happy for your care service to commence.

What Is Included In The Service?


Company


As part of our social companionship service, your Support Worker will spend time with you, whether that is spent sitting and chatting, looking through old family albums, watching television, or going out for short walks, they will be able to provide the company and reassurance that you need. Having someone to sit and reminisce with may be just the thing you need to brighten up your day.


Daily Routine


If you have trouble keeping track of what you need to do and when, your support worker can support you with your daily routine. They can provide you with reminders to do things, such as when to take your medication, to attend Doctors appointments, to write Birthday cards, to feed the dog and many other useful things that help you to remain independent.


Day Trips Out

Will my support worker help me with my pets?
Whilst our Support Worker will do everything they can to support you, we cannot guarantee support with your pets. If you do require help with looking after your pets, we will do our utmost to match you with a carer who feels comfortable doing so.


Minimum Visit Time

With our experience delivering expert care in the home, we know that even a brief visit to carry out your personal tasks is not something to be rushed if we are to carry out a safe and high standard of care. 
We strongly believe there should be no pressure to rush the tasks of daily living. We do not want to rush our Support Workers and we don’t want our Support workers to rush you.
During our visits, our care staff can offer support with not only personal care but also by providing company and conversation. We feel passionate about building strong, trusting relationships and believe this can only be achieved by investing time rather than delivering short bursts of care.
If you are simply seeking the reassurance of having someone visit your home to check you have everything that you need, we offer a flexible service with tailored visit-lengths which can be adapted to suit your needs. This can be with a carer visiting 2 or 3 times per week instead of daily or using their time to help you with other duties such as domestic chores or trips to the community. This ensures you get the most from your your time with your carer and a care plan than works for you.

Our Care Process


When arranging your care package, we follow a stringent process to ensure that you receive a truly bespoke and high-quality care service.


Stage 1
Our team receives your enquiry. We will discuss your specific care requirements with you and get to know your personality and preferences. We will provide information on the services that are available to you, as well as an outline of costs.


Stage 2
We will arrange for a member of our Team to visit you and carry out a comprehensive care assessment of your care needs, including a risk assessment to ensure your safety. Following this visit, a fully costed care package will be offered.


Stage 3
We will finalise your care plan with you and your family, making sure you are happy with every aspect.


Stage 4
We will identify suitable healthcare professionals to support you in-line with your care plan and arrange for you to meet them to ensure that all your needs are met. If any additional training is required to support your care requirements, this will also be coordinated.


Stage 5
Your service commences.


How soon can my care service start?
Upon receiving your enquiry we will arrange to visit you in your home and carry out a full assessment of your care requirements and a risk assessment to ensure your safety.


A personalised care plan will be produced for you, which we will finalise with you and your family, making sure that you are happy with every aspect. We will select Carers to provide your care based on your needs, personality, and preferences.
The commencement date of the package will be dependent on your needs, the organisations availability and funding arrangements.


Funding Your Care
Many people presume that receiving a bespoke care at home service is beyond their financial means; however, as more information about financing care becomes available in the public domain, care at home is becoming an increasingly popular option.
Funding for care can be a confusing subject area, however, by carrying out a little research and utilising resources available to you, your options become clearer and easier to understand.


Public Funding
You may be eligible to receive help with funding your care from the Government. You have the right to request a free care assessment from your local Social Services department. This can include a community care assessment and a means-tested financial assessment to see if you are eligible to receive support with the cost of your care.

Private Funding
Should you not be eligible for public funding, or would simply prefer to fund your care privately, you will need to source your care independently. Faced with so many different options and conflicting information, this can become both a time-consuming and overwhelming task.
